About End-of-Life Care Partnership

The End-of-life Care Partnership (formerly the partnership to Improve End-of-Life Care in Utah) is a coalition of individuals and member organizations that are combining their efforts and expertise to improve care for persons near the end of life and their families. Nearly 400 individuals representing more than 200 organizations throughout Utah are affiliated. The partnership is led by a volunteer steering committee.

The Partnership is sponsored by HealthInsight and received initial funding from the Robert Wood Johnson Foundation. The grant that supported the program for its first four years ended in 2003. The Partnership continues to rely on grants, membership fees and corporate sponsorship.

• From 1999 – to the present the Partnership has met these major objectives:
• Held Town Meetings to publicly discuss end-of-life care in Utah.
• Facilitated more than 25 “dialogue to action” focus group programs where families were brought together with healthcare providers to discuss end-of-life experiences and to encourage change in the way care is provided.
• Worked with KUED – 7 to publish and distribute 25,000 copies of a resource guide—“Living With Dying: End-of-Life Care in Utah.
• Publication and distribution of 15,000 “Toolkit for Health Care Advance Planning” workbooks.
• Publication of the Desert News “Final Acts” series on end-of-life care in Utah.
• Implementation of the Physician Order for Life-Sustaining Treatment (“POLST”) form.
• Development and maintenance of web site www.carefordying.org that receives thousands of visits every month.
• Conducts monthly educational meetings for both professionals and the public.
• Provides speakers to organizations interested in end-of-life issues and care.
