Welcome to www.carefordying.org, the web site for End-of-Life Care Partnership in Utah. The End-of-Life Care Partnership is a coalition of individuals and members organization that combine their efforts and expertise to promote dialogue with professionals and the public with the intention of supporting practices, which will improve care for persons, near the end of life and their families.

Vision: In Utah, people facing life-threatening illness and their families will expect and receive competent, responsive and compassionate end-of-life care.

Mission: We will focus our combined resources to provide Utahns education about palliative and end-of-life issues and choices.

Partnership Objectives:

Engage the public in conversations about end-of -life care.
Improve professional practices in end-of-life care.
Educate by providing patients and families with resources for end-of-life decision.
Advocate for improved end-of-life care by serving as resource to policy makers.

Your Rights: You have the legal right to make decisions about the medical care you want to receive, including whether or not to stop curative or life-prolonging treatments. In order for your wishes to be honored you must both document them and communicate them to your doctor and family.

Partnership Resources: The End-of-Life Care Partnership provides resources that explain your options, offer you information you need and to make decisions and direct you to appropriate care providers and support organizations.
